Room Rates:
The conference will be held at the Minneapolis Marriott City Center Hotel, 30 South 7th Street, Minneapolis, MN 55401. We suggest you make hotel and travel reservations as early as possible. A special conference rate of $172 per night is available.
Reservations:
Reservations should be made directly with the Minneapolis Marriott City Center Hotel at (612) 349-4000. Please ask the reservation agent for the "Quality Health Care Conference" discounted rate. Alternatively, call Marriott reservations at 1-800-228-9290. Please note this is the main Marriott reservations number, so you will need to specify city, state, hotel name, conference name and dates to reserve a room at the conference rate.

Payment Information
Purchase Orders are accepted. The PO number is REQUIRED to use this payment method. The PO must be received within 14 days of registering online or the registration will be cancelled. PO's must indicate the attendee's name. If the PO is not paid before the conference begins, a credit card will be required to ensure payment upon check in.

All Payments and Purchase Orders are due within 14 days of registering for the conference or the registration will be cancelled.
Early Bird Deadline is August 21, 2008. Online registration deadline is September 5, 2008. All registrations must be completed on site after this date.

Refund Information:
Cancellations: We reserve the right to cancel this activity for any reason whatsoever. In the event of such cancellation, the full registration fee will be returned to the registrant. Participants may cancel their registration prior to August 31, 2008 for a full refund, minus a $50 processing fee. After August 31, 2008 registration fees will not be refunded. Substitutions can be made at any time.
All cancellations and substitutions must be made in writing. All refunds will be issued after the conference.
