Reservations:
Due to the popularity of the conference, the Minneapolis Marriott City Center Hotel has sold out of rooms. We currently have a block of sleeping rooms available at the nearby Residence Inn, located one block from the Marriott, for a rate of $165.00 per night plus tax. Please call the hotel directly at 612.677.1000 and ask for the Quality Health Care Conference rate.

Payment Information
Purchase Orders are accepted. The PO number is REQUIRED to use this payment method. The PO must be received within 14 days of registering online or the registration will be cancelled. PO's must indicate the attendee's name. If the PO is not paid before the conference begins, a credit card will be required to ensure payment upon check in.

Early Bird Deadline is August 21, 2008. Online registration deadline is September 5, 2008. All registrations must be completed on site after this date.

Refund Information:
Cancellations: We reserve the right to cancel this activity for any reason whatsoever. In the event of such cancellation, the full registration fee will be returned to the registrant. Participants may cancel their registration prior to August 31, 2008 for a full refund, minus a $50 processing fee. After August 31, 2008 registration fees will not be refunded. Substitutions can be made at any time.
All cancellations and substitutions must be made in writing. All refunds will be issued after the conference.
